Registration for all Religous Education Classes in English is taking place
To aid with registration, all forms are available here online.
Simply print and complete the needed forms and bring them to the parish office
The following 4 things are needed to complete registration :
1) Completed FAITH FORMATION Registration Form
one form per family
2) Completed CONSENT TO PARTICIPATE Form
1 form per youth registering in grades 7 thru 12
3) Completed YM Activities & R.E. Fee Schedule
outlines Book, Program, Activities Fees
4) A COPY of Baptismal Certificate for each child registered
attach copy to Registration Form
If you have already provided the Religious Ed office with these copies, you do
not need to bring additional copies. If your child has received Baptism at Sacred
Heart or another church and you do not have the certificate, parents are asked
that prior to registration, please request the certificate(s) from the parish Records
Keeper of the church of baptism
